# General

• We will provide dental care on a private basis only and estimates of treatments will be given by dentist in writing before treatment is carried out.
• Additional procedures will be carried out if essential and in the best interest of patient after obtaining the patient’s consent.
• In some cases the modified treatment plan may involve extra charges.
• Estimates given to patients will remain valid for a period of one month from the date of the estimate or as otherwise specified in the estimates.
• We request that fees are payable before or on the day of treatment.
• Long period of treatment we reserve the right to request full or partial payment of fees before the day of treatment.
• We reserve all right to discontinue treatment at any time without any obligation for compensation in some condition of patient.

# Methods of payment

• For patient convenience we accept cash, cheques.

# Fees

• We require fees to be settled at the appointment where treatment is provided.
• We reserve right of 5% above on overdue fees remaining unpaid at the end of treatment.
• We also reserve right to make charges for returned cheques.
• In case of treatment provide under insurance schemes or finance agreements the patient remains liable for payment.
• Normal fees for consultation appointment.

# Emergency appointments

• We request that if you need to see a dentist urgently you contact umiya multispeciality dental clinic and cosmetic center as early as possible.
• We will respond you as soon as possible.

# Unattended or Cancelled appointments

• We understand that you are not always able to attend a previously scheduled appointment please contact us and we will give you to rearrange to your convenience time.
• We reserve right to charge full fees due failure to attend an appointment or if insufficient notice is given.
• We reserve all right to cancel appointment without prior notice in some condition.

# Consent forms

• Some treatments require completion of a consent form (e.g. teeth whitening, dental implant, dental impaction, and dental intravenous sedation) we will explain the treatment, complication, after care and some risk to you. Treatment cannot proceed without the signed forms.

# Personal details

• It is very important that you give a full medical history and dental history, any medication you take. When you change these it is important to tell dentist and give current information.
• We reserve all right for those patient who do not give proper information.

• We reserve all right for confidential information about patient we do not permit any person to make use of or divulge any confidential information otherwise than in the proper performance of the services or as required by law.

• We reserve all right prior to beginning treatment, the guarantees terms will be reviewed with you depends on dental science.
• The aesthetic and functional performance of work done in your mouth will depend on the workmanship of the dental laboratory work involved as well as on condition of your teeth, gums and bones, dental science.
• Factor affecting length of guarantee period includes oral hygiene, smoking, diabetes, bone density and diet.

# Data protection Act

• We store mostly all patient personal details on a computer system in accordance with the data protection act.
• All clinical notes, digital radiographs, digital photographs etc remain property of umiyamultispeciality dental clinic and cosmetic center. Copies of notes, radiographs, and photographs can be made available on request.
• We reserve the right to make an extra charge for any copying.
